We offer a set of home care services through our visiting nurse program (VNP). The VNP serves those who are homebound and have home care service needs that are medically reasonable and necessary to the treatment of an illness or injury.

How do I get started?
To ensure that VNP services are right for you, talk to your doctor about your home health needs and ask about VNP services.

Make an appointment with your doctor. If VNP services are right for you, contact VNP services at Southern
Humboldt Community Clinic by calling (707) 923-3921, ext. 273 and ask to speak to a VNP nurse.
Are VNP services covered by insurance?
Contact your insurance company to verify VNP services are covered. Ask the VNP nurse if your insurance is accepted at SHCHD.
Only a physician may order home health services and certify other eligibility criteria are met (medically necessary and homebound status). The physician must have seen the person needing care within 90 days prior to or within 30 days of, the start of home health.
VNP program referrals will be reviewed for qualifying criteria and accepted on a case-by-case basis.
Upon approval, an appointment will be scheduled to complete an in-depth assessment of current health status. Once completed, the VNP will develop a plan of care with the physician and care recipient. Once approved, the VNP can provide services.
